Home Health
The Home Health Department is located at Kit Carson County Memorial Hospital. Our Home Health provides skilled nursing, home health aide and physical therapy to Medicare, Medicaid, Workman’s Compensation and private insurance clients. All patient care is coordinated and supervised by a registered nurse and is provided in the comfort of the patient’s home.

Monday - Friday 8 am - 12 pm & 12:30 pm- 4:30 pm
Home Health Services Include
Skilled Nursing (perform technical procedures, assessments, observation and instruction)
Physical Therapy (PT assist in motion restoration through a variety of exercises and therapies)
Home Health Aide (assist patients with personal care)
Infusion Care
Wound and Ostomy Care (new wound, wound vac, new ostomy)
Pre and Post-Operative Care
Disease Education (Difficulty managing diabetes, HTN, CHF, etc. Increased shortness of breath)
Medication Teaching (New or changed medications)
Injection Administration (of Medicare approved medications)
Catheter Care (PICC, urinary, etc. management/dressing changes)
Reducing length of stay in a hospital or nursing home (recent discharge from hospital/nursing home)
New medical condition or diagnosis requiring teaching and training
Tube feeding management
Observation and assessment
Complex care plan management and evaluation
